Osteoporosis: Weights Friend, Not Enemy
When people hear the word osteoporosis, their mind immediately goes to calcium. Although calcium is certainly important, there are absorption issues, and maintaining healthy bones requires an integrative approach rather than relying on a single nutrient.
Bone health depends on a combination of proper nutrition, resistance-based exercise, and healthy lifestyle habits, all of which work together to maintain bone strength as we age.
Osteoporosis is quite scary as it develops silently. Many people do not know they have it until they experience a dramatic fracture after a seemingly minor fall.
The good news is that there are evidence-based strategies that help to preserve bone density, lower fracture risk, and maintain independence throughout life. Bonus is that everything that helps with osteoporosis also helps you stay healthy, lean, and strong!

Understanding Osteoporosis
A quick definition of osteoporosis: a condition that is characterized by loss of bone mineral density (BMD) and deterioration of bone structure. This causes the bones to become weak and more susceptible to fractures (Rizzoli and Chevalley).
DEXA can often be used to diagnose osteopenia and osteoporosis. The hips, spine, and wrist are the most commonly affected areas.
Before osteoporosis, many individuals experience osteopenia, which is just an earlier stage of bone loss. Here, bone density is lower than normal but not quite severe enough to meet diagnostic criteria for osteoporosis. Very good news: if you have osteopenia, you can certainly start modifying and improving your long-term outcomes.
Women are often more affected than men; however, neither party is immune. Risk factors include:
Aging
Menopause
Family history
Smoking
Excessive alcohol intake
Physical inactivity
Low body weight
Certain medications
Nutritional deficiencies
Although some of these risk factors cannot be changed, many can be addressed with lifestyle modifications.
Nutrition: The Foundation of Strong Bones
Okay, calcium will always be the first thing you see when researching osteoporosis, so we want to talk about a few other important things.
Keep in mind that calcium is needed for bone formation, but vitamin D is highly beneficial, helping the body absorb calcium efficiently (Nelms and Sucher). High-risk individuals for osteoporosis should ensure they meet recommended intakes of both nutrients through their diet, sensible sun exposure, and supplementation, particularly vitamin D in the upper hemisphere.
Magnesium is another essential mineral for vitamin D metabolism and bone mineralization. Vitamin K2 helps direct calcium into bone rather than soft tissues. Certain trace minerals, like zinc, copper, boron, manganese, and silicon, help contribute to collagen formation and normal bone remodeling.
Although we don't discourage supplementation, diet should also be the main driver, following a Mediterranean-style eating pattern rich in fruits, vegetables, legumes, nuts, dairy, lean proteins, and healthy fats. It provides many of the nutrients associated with better bone health and a lower risk of fractures, while also helping individuals stay metabolically healthy and lean (Rizzoli and Chevalley).
Protein: An Overlooked Bone-Building Nutrient
Roughly one-third of bone is comprised of protein, mostly collagen. This provides the structural framework that minerals strengthen.
Protein supports muscle maintenance, balance, fracture healing, and the production of insulin-like growth factor 1 (IGF-1), a pivotal regulator of bone formation (Kędzia et al.).
In the past and still to this day, I hear some individuals discourage high-protein diets because they were thought to increase calcium loss. However, evidence suggests the opposite: as long as calcium and vitamin D intake are adequate, more protein supports bone density rather than harming it (Wallace and Frankenfeld).
Experts now recommend that older adults consume approximately 1.2-1.6 g of protein per kg of body weight per day, particularly if they have osteopenia, osteoporosis, or age-related muscle loss.
Distributing protein evenly across the day, in meals of around 25-30 g each, is best to maximize muscle protein synthesis while also supporting both bone and muscle health (Rizzoli and Chevalley). Yes, women, particularly older women, should be eating just as much, if not more, protein than many high school male athletes (bodyweight equated).
Iron: The Missing Piece of the Bone Health Conversation
You'll rarely hear of iron when discussing osteoporosis prevention, yet it plays several important roles in maintaining healthy bones. Iron is necessary for:
Collagen synthesis
Delivery of oxygen to bone-forming cells
Normal osteon function
Iron also contributes to vitamin D metabolism and overall bone remodeling (Pizzorno et al.). If iron levels are low, these processes cannot take place optimally, which can contribute to lower bone mineral density and increased fracture risk.
Iron deficiency is quite common among women, older adults, and individuals with gastrointestinal disorders, as well as those following overly restricted diets. Rather than taking an iron supplement, it's recommended to evaluate iron status with laboratory testing, such as ferritin, serum iron, transferrin saturation, and a complete blood count, which is generally quite easy to obtain from your doctor.
Excellent dietary sources of iron include lean red meat, shellfish, beans, lentils, spinach, and pumpkin seeds. Pairing plant-based iron sources with foods rich in vitamin C can improve absorption.
Just remember that you could eat boatloads of spinach and still not get as much iron as in a single steak. This is due to heme iron from meat versus non-heme from vegetables. Heme iron is far superior to non-heme iron in absorption.
Why You Shouldn't Be Afraid to Lift Weights
One of the worst yet biggest misconceptions around osteoporosis is that people with weaker bones should avoid heavy weight lifting.
Although this fear is quite understandable, and there's a lot of bad advice via videos, social media, and online courses helping people with osteoporosis; they are often scared to recommend weight lifting to the general public, as it could potentially cause injury and therefore lawsuits.
We need to understand that bones respond to mechanical stress. When muscles pull against the bone during resistance training, specialized cells in the bone receive signals to strengthen the skeleton. This concept, known as the mechanostat theory, explains why appropriately loaded bones become stronger over time (Hong and Kim).
Research consistently shows that progressive resistance training is one of the most effective non-pharmaceutical interventions for maintaining or improving bone mineral density (Hong and Kim). This also improves muscle strength and balance, enhances confidence in movement, and lowers the likelihood of falls (the leading cause of osteoporotic fractures).
Exercises like squats, lunges, step-ups, resistance machines, and even deadlifts, believe it or not, provide beneficial loading to the hips and spine, the most vulnerable areas to osteoporosis.
People with osteoporosis should follow individualized resistance training programs, just as everyone else does. Osteoporotic patients should begin lifting light and slowly walk their way up under guidance from a professional when possible.
The goal should never be to avoid movement. It should be to move safely, wisely, and consistently, while also increasing load over time.
A Comprehensive Approach to Bone Health
Although medications are appropriate for some individuals at high fracture risk, lifestyle intervention should remain the foundation of osteoporosis prevention and management (Rizzoli and Chevalley; Nelms and Sucher).
An osteoporosis comprehensive plan should include:
Regular resistance training at least 2 days per week
Adequate dietary protein
Sufficient calcium, but not too much, and vitamin D intake
Careful attention to iron status
Smoking cessation
Moderate alcohol consumption
Balance training
Adequate sleep
Stress management
Together, these are beneficial habits that support healthy bone remodeling while also helping to preserve muscle mass and functional independence as we age.

Next time someone talks to you about osteoporosis, inform them that there is more to maintaining strong bones than calcium.
